We service customers in all 50 States in the U.S. So if you cannot make it to one of our locations to drop off your  tax returns, you can just simply print off the "Client Information Sheet" below and  either Mail, Fax, Email, or Drop Off  your tax return information to the kind people at J & W Services. 

 

Along with the "Client Information Sheet", you will need to include the following items:

• Wages Form

• A Copy of your State ID

• A Copy of your Social Security Card ( also include your dependents )

• Your Bank Routing Number and Bank Account Number ( for Direct Deposit )

 

 

If you are filling Self-Employed, you will need to send  us the following items:

•  "Client  Information Sheet"

• A Copy of your State ID

• A Copy of your Social Security Card

• A Copy of your Business License or Certificate

• Your Bank Routing Number and Bank Account Number ( for Direct Deposit )
